BURTON Albion Community Trust (BACT) is pleased to share the news that from this October it will be
working alongside colleagues from the East Staffordshire Primary Care Network (PCN) to develop,
deliver and lead on Social Prescribing across East Staffordshire.
Social prescribing is a core part of the NHS approach to delivering targeted Personalised Care across
England.
The practice, known as ‘social prescribing’, has been shown to improve patients’ quality of life, health
and wellbeing by treating them holistically, recognising that health is affected by a range of social,
economic and environmental factors.
Working together, BACT and the PCN will connect people to initiatives in their local communities to
improve their mental health and wellbeing in response to the impact of COVID-19, they will also
develop a holistic approach and build support by professionals who can help individuals focus on
what matters to them, to live well.
Social prescribing supports people to understand their needs and connects them to local community
(non-clinical) often voluntary services which can provide the help they need.
The partnership will see 5 social prescribers work across East Staffordshire, working alongside
colleagues from GP practices, VCSE partners and other groups who can support individuals physical
and emotional wellbeing.

social prescribers across the area working with our GP’s, Care Coordinators and the local community
will strengthen the offer and make a real difference across East Staffordshire.”
